Scheduling Appointments
We see patients from birth to 21 years of age. Appointments are required for all physicals, new patients, immunizations and flu shots. Please note that well visits cannot be done at the same time as a sick visit and should be scheduled separately. We offer walk-in hours for established patients only and same day sick appointments. Please call as early as possible in the day to schedule a same day appointment for sickness.

Payment Policies
Payment is due at the time of your child’s visit. This includes co-pay’s, balances from prior visits, and payment in full if you are self pay. Medical coverage is between the insured and the insurance company. You are responsible for knowing the details of your coverage.
Request of Medical Records
If you need a copy of your child’s medical record a parent or guardian must sign and date a medical release form. There is a $15.00 processing fee per chart, $0.50 cents per page for 21-50 pages and $0.25 cent per page beyond 50 pages. There is no fee for a copy of ONLY the immunization record. We do not charge for doctor to doctor request. If you have not been seen in our office in the past few years, your child’s chart may have been placed in storage. There is a $22.00 fee for retrieval. Regulations of the Virginia Board of Medicine state that practitioners must maintain a patient for a minimum of six years following the last patient encounter with the exemption of records a minor child, including immunizations, must be maintained until the child reaches 18 years of age with a minimum time of record retention of six years from the last patient encounter.
